I am aware that there is a certain degree of risk involved in all physical movements. It is the intent of the Baby Ballet Academy teacher, to provide a safe environment and correct training in order to avoid any injury to the student enrolled.

Therefore, I agree to release the Baby Ballet Academy from any liability, should my child become injured while under the supervision of the Baby Ballet Academy teacher.
